Peritoneal Ventilation for Adult Respiratory Distree Syndrome in Rabbits

Abstract

|
Adult respiratory distress syndrome is the first manifestation of multiple organ failure and brings about the sequential failure of other organs. Hypoxemia is the major problem, but in many cases it is refractory to conventional mechanical
ventilation
and many complication are possible. For refractory and prolonged hypoxemia, extrapulmonary oxygenation has been attempted and extracorporeal membrane oxygenation is the representative. Peritoneum has a large surface area and the ability of
absorption
can be served as the possible route of extrapulmonary oxygenation. We tried peritoneal ventilation in hypoxemic rabbits. We inducd acute respiratory distress syndrome with saline instilation (10ml/kg) through an endotracheal tube in the rabbits.
In
the
control group, controlled mechanical ventilation was done. In study group, intraperitoneal pressure was kept at 3 cmH2O, and peritoneal mechanical ventilation with 100% O2 was added. But we failed to improve hypoxemia with the additional
peritoneal
ventilation. We think that the pulmonary mechanics in relation to increased intraabdominal pressure and the absorptive ability of peritoneum needs further studies.
